Butcher屠宰场
出医院后到屠宰场（图289），路上溜边走有很多东西可拿。屠夫（开始看成三角头了）出场，在Travis面前将护士从下腹部劈成两半（图113）。这里可取得药水和菜刀。

Sanitarium疯人院
从屠宰场出来进入大地图右下的疯人院，先去West Solarium 房间一个有人偶的椅子上拿到手枪（图118），再去East Solarium房间取这里的地图以及一份文件。从East Solarium的另一门出来碰上Alessa的母亲Dahlia，她说是她放火烧死自己的女儿Alessa，Travis救Alessa就是多此一举。Travis正想问她为什么会知道自己的名字，Dahlia转身走了。前面storage找到Tire Iron和Screwdriver，剩下的门不是锁着就是坏了，于是走右面的楼梯上2层，地下室锁着进不去。

影子怪出现，把收音机和灯一关，听其脚步声躲开，不用管它进入storage，取得Alcohol bottle，散弹枪子弹和急救包。回到有影子怪的地方左走，路上注意护士的攻击，个人习惯QTE后直接拳头解决。TB Ward的锅炉样机器谜题要求把仪表上所有的指针同时指向红区，按照135135的顺序关掉锅炉取出里面的钥匙。现在可以地下室可以打开了，若进左边的门storage有件冷兵器。看见一怪物向左走去跟上去，最里面房间是污水处理场，调查绿色排水孔提示所有的污水都汇集在这里（后面要到这取钥匙）。出门走向上的一分路，1层被锁，进入2层。Female hydrotherapy有存盘点，发现水疗盆里有把钥匙（图120），打开墙上的机关，钥匙被冲走。Female treat屋中利用镜子返回里世界，调查床上人偶获得子弹。出门后只有楼梯可通行。

此时的疯人院已面目全非。里世界一层左边发现奇怪标志（图121）。右走场景突然变灰，一段提及Helen和她儿子的奇怪对话后世界恢复其残破模样。女厕所里可回到表世界，厕所中有一个被铁丝封住了，进不去，最里面还有把菜刀别忘了。回到表世界女厕所，在刚才有铁丝的地方选择冲洗。里面的地面上写着我要我的儿子Bring me my son。出门向上到Female Dorm 5，除了meno还有警棍baton，子弹，并在床上的吊栏里得到patient belongings钥匙（图125）。patient belongings在中央大厅的上边，进入后发现一份病例报告，医生Harris说病人最近沉浸于其幻想的“镜世界”中，并已经回想起自己曾毒打儿子，不但没有懊悔还甚为自豪。拿走这里的子弹、打印机进入镜子，终于在门上找到散弹枪。里世界的走廊已经扭曲不堪，Day room的轮椅上有子弹，急救包和棍子Poker，East Solarium里取走散弹枪子弹和Filing Cabinet。Laundry里发现一份报告书，说Helen想开煤气杀死她儿子，他是恶魔之子。

下楼到地下室的East Pipe Room去捡先前冲下来的钥匙，下面标示Interview Archives key，Archives房间在2层的中央大厅（图130）。返回到楼梯口处的Storage房间回到表世界，顺便拿走架子上锤子和Ampoule（安瓿是在药物注入患者体内之前贮存药物的适宜的包装材料）。进入Archives房间又听到一段Helen和Dr.Harris的对话，由此前面的匿名的Note也可得知是Helen的病例。Helen说她儿子的体内隐藏着恶魔，是个坏小孩，必须要受到惩罚。既然是她将他带到世上，也应该由她将他杀死。她说她可以随时进入镜中的另一个世界，而现实的世界只是白日梦。顺走电视和散弹枪子弹后进入里世界，出门前带走架子旁的武士刀Katana。

中央大厅只有左边能通过，下楼梯进最底层的大门，附近的storage有镜子。先继续在里世界前进到地图左上的楼梯至1层。参观下男病人隔离区Man Seclusion（图133），在椅子上发现兵器和药单。5号房估计是个禁食者，桌子上的食物都没动过，还有一个体重计。4号房门被铁栅栏挡着进不去，是个杀人狂。3号里可以见到2代经典场景（图136），只不过这个角度太平常了。2号的门烧焦了更进不去，纵火狂。1号调查桌子找到剃刀Razor，上面满是血迹，自杀狂。医务室Infirmary有存储点和人偶谜题，还记得在隔离室的那个药单么，按照绿蓝蓝红黄把药片放入人偶口中（图137），得到Dr.Harris的钥匙。

原路返回地下室有镜子的storage，回到表世界。桌子上的Note显示有个男孩溜进疯人院的女隔离房，还好及时发现并让其父亲带走。来到1层进入Dr. Harri’s office（图140），拿到桌上的Jocasta artifact（形状和前面有个门一样，图139）和一张字条，字条上说Helen看起来很平静无害，但看完有关她的纪录后“我”开始觉得很棘手，她一遍遍的要见她儿子，我和Helen的丈夫认为最好让他们分开。

终于见到Helen，没想到她竟是Travis的妈妈。然而此时Helen已经变成丑陋的怪物。BOSS战很简单，散弹枪子弹够充足的话很快就可以解决，BOSS的攻击不是放毒就是放刀片，都是近身的，所以拿枪跑动打游击才安全（图153）。胜利后得到Past Piece。Alessa又出现了，Travis问她这是怎么回事，是不是她干的，Alessa什么也没说就消失了，而Travis又晕了，醒来后已在疯人院的入口大厅内。

把票放入检票口（图171），大门打开。厅内Box office前取得剧院地图，右边有存储点。观众席门前告示牌上取得Theater Program page，进入后又见Lisa。Lisa说她喜欢剧院，因为她曾经梦想成为演员，但其母亲是护士，母亲的母亲也是护士，所以她不情愿也成为了护士。突然Lisa妩媚的靠近Travis轻声说Love him，一下子把Travis弄得飘飘欲仙，最后Lisa大笑一声说我演的不错吧，Travis愣了下才返过神来说了声Yeah……（图177）

回到表世界，经过大厅走左边上2层打开balcony corridor房门，进入Lighting box分别找到125W、750W、250W、500W灯泡，桌上得到Eric的解迷便条。右边下楼在房中间（图185、186）的展台上取得月亮图腾。回到女厕所去里世界，上2层把刚得的月亮图腾以及太阳图腾放入门前的方形缺口中，进入星形门。上楼梯后发现一大怪堵道，杀之。从Costume storage回表世界之前顺着门外的路走到尽头拿到来复枪Hunting Rifle（图187）。在表世界的Costume storage取得武士刀，药水和纸条。左边Orchestra storage里有储存点和Iron weights。走最右边的路，第一个catwalk门前有来复枪子弹，进入第二个catwalk到灯泡解谜处。

调查每个聚光灯得知A、B、C、D的位置，灯泡放置要求为：B为D的一半，A+B小于C，D的瓦数小于A。所以，A为500W，B为125W，C为750W，D为250W。放好后拉下尽头的开关。回到一层的curtain control拉下门口的开关。此时舞台上出现一面大镜子，右边有布景控制器（图190），不过右边第三个控制杆没了。调动左1，右2，树+树林，进入镜子在林中左边的红色树上找到stage office key。调动左3右1，桌子+书架，进入镜子。Travis发现这里书中的文字很奇怪，书也不是纸制的。桌上有3篇有关精神的文章，左边的台子上有子弹及另一篇文章。接下来去地图上方的stage office，解决一个人偶后拿走桌子上的Control Lever。回到布景控制台，使用Control Lever，调动左2，右3，进入镜子。

General Store
除了血瓶和弹药外还有一张警告字条：来吧点燃你的床，来吧让屠夫砍掉你的脑袋。

出门向右走中间的小路躲过一个大怪后向下（图208），剧院的BOSS此时作为小兵挡道。路边有不少补给，注意拾取。来到地图画圈处进入栅栏门。

进入Motel后Travis回忆到自己和父亲在这里住过。桌子上有Mr.Gragy和儿子的入住登记（就是Travis和父亲，住在500房间），鹿头旁拿走旅店的地图。前台后面取得306房间的钥匙，前台旋转日历暂时操作不了。向下来到住宅区，1层302房有一床的靴子。209房除掉吐酸怪后拿走来复枪子弹。上2层，316里可取得木棒。312有子弹补给和吐酸怪。308里有吐酸怪的变体及子弹。下楼进306房间，进入锈迹斑斑里世界（图215）。此时只有305房上面的门能进（图216），来到30X和20X区，301有一急救包和吐酸怪。209有冷兵器长矛。中央还有叉子Pitchfork可拿，进503回表世界。房间里的字条说明了巨大的精神能量能够作为召唤邪神Samael的容器。

Maintenance Room里注意吐酸怪，进入狭长的Maintenace。通过墙上的洞可以看见在500房间Travis的父亲极其想念妻子Helen，每当看见儿子就像看到了Helen，并说要夺回偷走妻子心的东西。注意墙上挂历上红色的数字12（接待处谜题答案之一）。路上拿到Broken Pole，最后看到拿刀的屠夫（图222）。Manager’s office钥匙和照片在桌子上找到。进入staff accommodation左边的门（图223）绕到地图右上的Manager office，一张纸上说500房出了些问题，恶臭冲天，一个男孩在那站了 10个小时，警察已经封锁了房间（看来父亲的死对男孩<就是Travis>的打击不小）。此时可以去地图上面的108房，灭怪后取得Redeemer和子弹。106房同样灭怪再取菜刀。

Kitchen里与屠夫BOSS交战。绕圈和他周旋再放几枪，门旁的肉钩可以利用，被其抓住后两次QTE即可逃脱。Diner房间茶几上有Travis父亲Richard给妻子的信，说他所爱的Helen已经死了，但她的躯体还存在！Richard孤枕难眠已经2年，变得越来越痛苦和堕落。他已不能尽父亲的责任，甚至有时看Travis都痛苦万分。在Game Room的弹珠台取得一硬币，台球杆都可以拿走，出门时出现回忆。

Laundro mat，中间烫衣板取得操作守则，投入刚才取得的硬币启动洗衣机。翻看Note得知操作顺序，对应操作守则旋转按钮再按下即可：Low spin，60 degree wash，drain，per-rinse。洗衣机打开后得到Cleopatra key。102房和201有怪物和些补给。上2层进入Cleopatra房间（图228），床上有照片，跳下浴室的洞后看到Lisa和医生Kaufmann从床上起来。从桌上Dahlia留下的字条得知召唤Sameal 的仪式就要开始了。进入屋中镜子的里世界，一枪解决怪物后取下铁网上的相片，这时可以拿水池中央的宝石心了（地图中有心的标识）。Dinner里取得仪式用剑ornamental dagger（图234），插入2层的Nero房间的门。又是通过洞下至King Suite房间，再进入墙上的洞到Maintenance。先向右取得照片，再进Maintenace room找到有封Richard给Helen的诀别信。从503房间回到表世界，到Maintenace room（图239）用工具将宝石心磨碎，得到结婚戒指。调查戒指，得到六月这个信息。

回到接待处Reception，把旋转日历调成6月12日1961，机关打开，放入结婚戒指，转身在木板上取得500的钥匙。回里世界打开500房间的大门，通过长长的血色通道，终于见到上吊自杀的父亲（图243），也记起了这段自己不愿回首的往事（母亲想杀儿子，父亲百经心灵折磨自杀）。BOSS也很简单，用Hunting Rifle等枪乱射后轻松干掉。得到truth piece后Alessa出现，Travis再次晕倒。醒来后捡到present piece。将得到全部piece拼成一个，规律是找与中心三角其相邻三个面相似的拼上，达成每面是一类型图案（见下图254-257）。

通关后存档再继续游戏就会发现多了些东西，像夜视镜，各种服装等。
Option里开启Accolades和Extra option选项
Accolades里面是你在游戏结束后取得的称号，一共14个。比如多杀怪、多探索地图、经常关闭手电都有相应的称号。
Extra option可更改跑步/走路模式，消除画面上的上的杂质，改变手电射出光束形状等。
